Sacraments of Healing​

If you need a copy of your Sacramental records you will need to submit a Sacramental Records Request Form to Jen in our parish office. Please download the form, (it is a word document) then save and attach in an email to westpointstmary@diodav.org or print and send to the parish office. The Diocese of Davenport no longer allows us to take verbal requests for copies of sacramental records.

Anointing of the Sick:  If you or a loved one are suffering from a serious illness, or if health is seriously impaired by age, a person ought to be anointed. Should one’s condition worsen, the sacrament may be repeated. Serious mental illness may be a reason for anointing, as is upcoming surgery for a serious cause.
