If you need a copy of your Sacramental records you will need to submit a Sacramental Records Request Form to Deb in our parish office. Please download the form, (it is a word document) then save and attach in an email to westpointstmary@diodav.org or print and send to the parish office. The Diocese of Davenport no longer allows us to take verbal requests for copies of sacramental records.

Baptism: Baptism is arranged by completing the Baptism request form and submitting it to the parish office. You may email this form or drop it at the parish office. Once Father approves the baptism date it is officially scheduled.

First Reconcilliation: All students are required to have 2 consecutive years of religious education, either in Religous Ed or at Holy Trinity, when preparing for the Sacraments of Reconcilliation. Students should be enrolled and actively participating in classes for one year prior to the year he/she prepares for the celebration of the Sacraments. Students and their parents will participate in a First Reconcilliation Meeting in October. First Reconcilliation is usually celebrated in February during second grade. Students and their parents are required to make a confession during this service.

First Communion: Each year Students and their parents will participate in a First Communion meeting in January. Followed by the retreat and First Communion which is generally celebrated in the early Spring for second grade students. All students are required to have 2 consecutive years of religious education, either in Religious Ed or at Holy Trinity, when preparing for the Sacrament of Communion. Students should be enrolled and actively participating in classes for one year prior to the year he/she prepares for the celebration of the Sacraments.

Confirmation: Students in 8th grade will prepare for Confirmation both by completing class work and completing a series of Local Mission work. This program is designed to allow students to put their faith into action and model the Christian concept of service that Jesus taught his disciples. Confirmation is held in the spring of each year at one of the local area parishes. All students are required to have 2 consecutive years of religious education, either in religious ed or at Holy Trinity, when preparing for the Sacraments of Confirmation. Students should be enrolled and actively participating in classes for one year prior to the year he/she prepares for the celebration of the Sacraments. Marriage: The union of man and woman in marriage was ordained by our Creator in loving care for God's children, and instituted by Our Divine Lord as a Sacrament in the care of His Church. In conferring the Sacrament of Matrimony upon one another in the presence of the Church's witnesses, a man and woman give themselves totally to each other, for as long as they both shall live.
In preparation for the Sacrament of Marriage a couple needs to contact Deb in the Parish Office at least 6 months prior to the desired date of Marriage to confirm the date is available. Then a Marriage Request Form should be submitted to begin the Marriage preparation process. At least one party must be a practicing Catholic and a registered member of the parish. The couple will be required to participate in the Sponsor Couple program or the Engaged Weekend Encounter. The couple will also need to meet with Father Dennis Hoffman on 3 separate occasions. However, their are certain marriages that are invalid from the start. The Annulment process is a judicial process to allow people who believe that their marriage was invalid to attempt to prove that fact. In an attempt to improve this process Pope Francis issued a document revising the marriage nullity process. Holy Orders: Holy Orders is the sacrament through which the mission entrusted by Christ to his apostles continues to be exercised in the Church until the end of time: thus it is the sacrament of apostolic ministry. It includes three degrees: episcopate(Bishops), presbyterate(Priests), and diaconate(Deacons). The root word for vocation is vocare, which is Latin for "to call." God has created each one of us for a particular calling or way to follow Him. Through the Sacrament of Baptism we become a child of God and thus called to live in holiness through love of God and neighbor. It is important to also remember that our true vocation does not come from the world, but rather is an invitation directly from God to follow Him. Annointing of the Sick: If you or a loved one are suffering from a serious illness, or if health is seriously impaired by age, a person ought to be anointed. Should one’s condition worsen, the sacrament may be repeated. Serious mental illness may be a reason for anointing, as is upcoming surgery for a serious cause.
